The Nuffield Council on Bioethics (2017)
Dr Kate Edgar and Dr Sara Dowsett (BAAPS recommended psychologists) work collaboratively to provide cosmetic surgery assessment packages for plastic and cosmetic surgeons working in the UK. Together they have a wealth of experience in the psychological assessment of patients considering engagement in cosmetic surgery.
Assessing for existing mental health conditions such as Body Dysmorphic Disorder (BDD) is essential for the psychological safety of your patients and their response to the surgical intervention you are offering them.
All pre-surgical assessments include full administration of various evidence-based psychometrics designed for routine clinical use in initial consultations with all prospective patients seeking cosmetic surgery.
Cosmetic psychosocial desires and expectations such as: “I want to feel more confident” are often key motivators in your patients' decisions to engage in cosmetic surgery. Clarifying these factors is key in identifying your patients' vulnerabilities and enhancing ethical, multi-disciplinary care.
The British Association of Aesthetic Plastic Surgeons (BAAPS) and the Royal College of Surgeons (RCS) recognise the duty of care to cosmetic surgery patients by promoting collaboration between surgeons and psychologists.
We will explore the ideas and motivations for surgery. There might be numerous factors that are contributing towards your patients' desire to change their appearance and defining these is an important part of their overall decision on surgery.
Engagement in cosmetic surgery is typically due to a struggle or dislike with an aspect of your patients' appearance. It is therefore important that their current and past body image concerns (and how these relate to their current decision to engage in surgery) are explored.
We will discuss your patients' current and past mental health experiences and their general emotional well-being. We will also explore their support networks and coping strategies. Doing so will help with recommendations for pre or post operative care.
We will discuss your patients' expectations of the proposed procedure, for example what do they hope will change as a result of the surgery. We will also explore with them the physical and psychological risks associated with surgery to ensure they are fully informed before they make a final decision.
We will ask your patients' about the pros and cons of the proposed surgery and we can explore any doubts or hesitations they may have. Before making a life-long decision on surgery, it is important that they have time to consider the psychological as well as the physical impacts of surgery.
We will produce a comprehensive and detailed report within seven days of your patients' assessment. If agreed by the patient, reports can be shared with you which are inclusive of any recommendations discussed.
